how did they get the scores wrong?

Human error. She wrote the scores down next to the wrong name.

Awful on all sides, the candidate loses a job offer, the person will beat themselves up for making a that scoring error (let's face it we all do make mistakes) and the successful candidate who was rejected on the basis of the wrong score feels that they failed.
