During the Nevada State Board of Pharmacy's October Meeting which took place on October 13th & 14th of 2010.
*Pages 5-6 denote the following:
Tim Lopez was present with his legal counsel, Kevin Murphy. Kevin Murphy and Carolyn Cramer drafted a stipulated agreement and it was presented to the board by Ms. Cramer. Mr. Lopez/Applied Pharmacy dba Remedy Rx via his own admission admitted that the factual accusations were true. At this time he was placed on probation for five years with conditions. He was to be on the premises of Remedy during business hours only and that there would be another pharmacist present to deal with activities regarding the sale and transfer of Remedy. Remedy also had 180 days to arrange for the sale/transfer of its business to another person or entity. On day 181 after the stipulated agreement was approved Remedy would have to surrender its license if the sale had not transpired by then.
Keith Macdonald moved to accept the stipulated agreement-- it was seconded by Beth Foster. The agreement was passed unanimously via the Nevada State Board of Pharmacy.
